Progesterone gets blamed for—and credited with—a remarkable number of symptoms online. Fatigue, spotting, poor sleep, anxiety, infertility, bloating, miscarriage, weight changes, and irregular periods are all commonly described as signs that progesterone is “too low.”
The reality is more complicated.
Progesterone is an essential reproductive hormone, but its level is supposed to change dramatically during the menstrual cycle. It is relatively low before ovulation, rises after an egg is released, falls before a menstrual period if pregnancy does not occur, and rises substantially during pregnancy. A progesterone level that would be completely normal on one day of a cycle could mean something very different a week later.
That makes timing crucial when progesterone is tested.
It also means that symptoms alone cannot diagnose a progesterone problem. Irregular periods, spotting, difficulty conceiving, fatigue, mood changes, breast tenderness, headaches, or sleep disruption can occur for many reasons. Polycystic ovary syndrome (PCOS), thyroid disease, perimenopause, pregnancy complications, medication effects, stress on the reproductive system, and several other conditions may produce overlapping symptoms.
Progesterone also has several legitimate medical uses. But those uses are not interchangeable, and taking progesterone simply because a symptom list suggests a “hormone imbalance” may miss the actual problem.

What Is Progesterone?

What Does Progesterone Do?
Calling progesterone simply the “pregnancy hormone” misses much of what it does.
It Prepares the Uterine Lining After Ovulation
During the first part of the menstrual cycle, estrogen stimulates growth of the endometrium—the tissue lining the inside of the uterus.
After ovulation, progesterone changes that estrogen-stimulated lining.
Rather than simply becoming thicker, the endometrium becomes more secretory and biologically prepared for the possibility that a fertilized egg will implant.
If implantation does not occur, progesterone production eventually drops and menstruation follows.
It Helps Maintain Pregnancy
If pregnancy occurs, progesterone does not fall at the end of the cycle.
Instead, hormone signals from the developing pregnancy maintain corpus luteum activity early on. Later, the placenta produces much of the progesterone needed during pregnancy.
Progesterone helps support the uterine lining, influences uterine muscle activity, and participates in immunologic changes that allow pregnancy to continue.
It Helps Regulate Menstruation
The rise and fall of progesterone is an important part of normal menstrual timing.

How Progesterone Changes During the Menstrual Cycle
Progesterone is not meant to stay at one stable level throughout the month.
That basic fact is essential when interpreting a test.
Before Ovulation: The Follicular Phase
The follicular phase begins on the first day of menstruation.
Estrogen becomes the dominant ovarian hormone as follicles develop, while progesterone generally remains relatively low.
A low progesterone result during this stage is therefore usually expected rather than evidence of a deficiency.
Around Ovulation
A surge in luteinizing hormone, or LH, triggers ovulation.
Once the egg has been released, the remaining follicular tissue becomes the corpus luteum.
That is when progesterone begins rising significantly.
After Ovulation: The Luteal Phase
Progesterone usually reaches its highest menstrual-cycle concentration several days after ovulation.
ASRM notes that luteal progesterone typically peaks about 6–8 days after ovulation.
This is why the timing of a progesterone test should be tied to expected ovulation and the next menstrual period—not automatically to a particular calendar day.

The corpus luteum has a limited lifespan.

What Does “Low Progesterone” Actually Mean?
“Low progesterone” is frequently used online as if it were a stand-alone diagnosis.
It usually isn’t that simple.
The first question when looking at a low progesterone result should be:
When was the blood sample taken?
Low progesterone could be completely normal if the sample was drawn before ovulation.
If it was drawn when progesterone was expected to be elevated, a low result might suggest:
- ovulation had not yet occurred
- the cycle was anovulatory
- the test was mistimed
- hormone production differed in that particular cycle
- another reproductive or endocrine condition affected ovulation
Progesterone is also secreted in pulses, not at a perfectly steady rate.
ASRM reports that levels in normally ovulating individuals can fluctuate considerably over only a few hours. That makes a single number much less precise than many online “optimal progesterone” charts suggest.
What Are the Symptoms of Low Progesterone?
There is no symptom or group of symptoms that can confirm low progesterone without appropriate clinical evaluation.
This is important because online hormone content often attributes a long collection of common symptoms to progesterone deficiency.
Clinical situations in which lower-than-expected progesterone may be relevant include:
- missed periods
- irregular menstrual cycles
- evidence that ovulation is not occurring regularly
- abnormal uterine bleeding
- difficulty conceiving
- selected pregnancy concerns
But symptoms frequently marketed as signs of “low progesterone”—including fatigue, poor sleep, headaches, mood changes, bloating, breast symptoms, anxiety, and decreased libido—are nonspecific.
A person experiencing several of them could instead have:
- perimenopause
- PCOS
- thyroid disease
- anemia
- pregnancy
- depression or anxiety
- medication effects

Testing and medical history are therefore much more useful than matching yourself to a symptom checklist.
What Can Cause Lower Progesterone Levels?
When progesterone is unexpectedly low, the underlying reason often involves ovulation rather than an isolated failure to manufacture progesterone.
Anovulation
If an egg is not released, the normal corpus luteum does not form.
Without a functioning corpus luteum, the expected post-ovulation progesterone rise may not occur.
Anovulation can happen occasionally even in people who usually have regular cycles. Repeated anovulation, however, may be associated with conditions such as PCOS, thyroid abnormalities, elevated prolactin, perimenopause, significant changes in energy availability, or other hypothalamic, pituitary, or ovarian disorders.
Polycystic Ovary Syndrome
People with PCOS often ovulate less frequently or unpredictably.

This does not mean that PCOS is simply a disease caused by progesterone deficiency.

Perimenopause
Ovulation becomes less predictable as menopause approaches.
One month may include normal ovulation and progesterone production; another may not.

Hypothalamic or Pituitary Causes
The brain and pituitary gland coordinate the hormone signals required for ovarian follicle development and ovulation.
Severe calorie restriction, substantial energy deficit, excessive exercise in susceptible individuals, major illness, or certain endocrine disorders can interfere with those signals.
If ovulation stops, progesterone production also falls.
Pregnancy Complications
A low progesterone measurement during pregnancy can sometimes occur in pregnancies that are not developing normally.
But one progesterone value should never be used by a patient to diagnose miscarriage or ectopic pregnancy at home.
Early pregnancy evaluation may require:

- ultrasound
- serial hCG testing
- repeat clinical assessment
ACOG notes that ultrasound and sometimes multiple hCG measurements may be required before early pregnancy loss can be diagnosed reliably.
What About High Progesterone?
Higher progesterone is often normal.
It occurs naturally:
- after ovulation
- during pregnancy
- while taking progesterone treatment
A test result therefore needs context before it is labeled abnormal.
Less commonly, higher-than-expected progesterone can be associated with certain ovarian or adrenal conditions, but those diagnoses cannot be made simply from vague symptoms.
As with low progesterone, there is no reliable “high progesterone feeling” that allows someone to determine their blood concentration from symptoms.
How Is Progesterone Tested?
The most common clinical test measures progesterone in a blood sample.
A healthcare professional draws blood from a vein, and the laboratory measures serum progesterone concentration.
The test may be used in selected circumstances to:
- determine whether ovulation probably occurred
- evaluate infertility
- monitor certain fertility treatments
- investigate some forms of abnormal uterine bleeding
- contribute to evaluation of a pregnancy problem
- investigate unusual hormone findings
The result is interpreted based on why the test was ordered and when the sample was taken.
When Should Progesterone Be Tested?
For fertility evaluation, timing is more important than memorizing a single cycle day.
The “Day 21 Progesterone Test” Is Not Universal
Cycle day 21 is commonly discussed because it may roughly correspond to the middle of the luteal phase in someone with a predictable 28-day cycle who ovulates near day 14.
That assumption does not work for everyone.
Someone with a 35-day cycle might ovulate much later. Testing on day 21 could therefore occur too early and produce a low value that is perfectly appropriate for that point in the cycle.
ASRM recommends that when progesterone is being used to assess recent ovulation, the sample should generally be obtained about one week before the expected start of the next menstrual period, rather than automatically on day 21.
What Progesterone Level Shows That Ovulation Occurred?
A properly timed luteal serum progesterone concentration above approximately 3 ng/mL provides presumptive evidence that ovulation recently occurred.
That result answers one question reasonably well:
Did ovulation probably happen?
It does not necessarily answer a different question:
Was the luteal phase “good enough” to support pregnancy?
Can One Progesterone Test Measure Luteal-Phase Quality?
Not reliably.
ASRM notes that progesterone secretion is pulsatile and concentrations can change several-fold within hours.
A single blood sample can therefore capture a temporary peak or trough.
For that reason, ASRM states that there is no minimum single serum progesterone concentration that defines normal or fertile luteal function.
This is an important distinction when interpreting fertility testing.
A low-looking value does not automatically prove “luteal phase deficiency,” and a high value does not guarantee fertility.
What Are Normal Progesterone Levels?
There is no one progesterone number that should be considered “normal” for everyone.
Reference ranges vary based on:
- menstrual-cycle phase
- whether ovulation occurred
- pregnancy status
- pregnancy stage
- laboratory assay
- progesterone treatment
- reason for testing
A laboratory report may provide different reference ranges for follicular, luteal, postmenopausal, and pregnancy states.
Those ranges should still be interpreted alongside the clinical circumstances.
A result that appears low compared with a luteal-phase range may be completely normal if the person has not yet ovulated.
Similarly, comparing your progesterone concentration with someone else’s result from a different pregnancy week, cycle day, laboratory, or treatment protocol is rarely useful.
Progesterone and Fertility
Progesterone is essential to normal reproduction, but that does not mean giving extra progesterone improves fertility in every situation.
Progesterone Testing Can Help Confirm Ovulation
A properly timed progesterone test can provide evidence that ovulation occurred.
This may be useful during an infertility evaluation when the clinician needs to determine whether cycles are ovulatory.
For many people with reliably regular menstrual cycles, extensive testing to prove ovulation may not always be necessary.
“Luteal Phase Deficiency” Is More Complicated Than One Low Result
Luteal phase deficiency, or LPD, is a proposed condition in which progesterone production or endometrial response is considered inadequate to support normal reproductive function.
Its diagnosis remains challenging.
ASRM notes that no single reliable clinical test has been established to diagnose LPD and that a single serum progesterone concentration cannot assess luteal-phase quality reliably.
This makes statements such as “your progesterone needs to be above X to become pregnant” overly simplistic.
Progesterone in Natural Conception vs Fertility Treatment
There is an important difference between giving progesterone routinely during an unassisted menstrual cycle and giving it as part of assisted reproductive technology.
Progesterone support is well established in many fertility-treatment protocols.
For example, FDA-approved vaginal progesterone products are available as part of assisted reproduction treatment to supplement corpus luteal function and support implantation in defined settings. The FDA updated labeling for one such vaginal progesterone product in 2026.
That does not mean every person trying to become pregnant benefits from taking progesterone.
The treatment needs to match the fertility protocol and underlying clinical problem.
Progesterone During Pregnancy
Progesterone plays a central role throughout pregnancy.
Early in pregnancy, the corpus luteum produces progesterone under hormonal stimulation from the developing pregnancy.
As placental development progresses, the placenta takes over much of the progesterone production.
The hormone helps maintain the endometrium and contributes to the uterine environment necessary for continued pregnancy.
Because progesterone is essential to pregnancy biology, it is easy to make an incorrect leap:
If progesterone supports pregnancy, then taking more progesterone must prevent miscarriage.
Clinical evidence does not support that conclusion for every pregnancy.
Can Progesterone Prevent Miscarriage?
Not in every situation.
Miscarriage has many causes, and progesterone can only influence some clinical scenarios.
ACOG notes that approximately half of early miscarriages occur because the embryo developed with an abnormal number of chromosomes. Increasing progesterone cannot correct those chromosomal abnormalities.
Evidence does support progesterone treatment in some selected patients.
For example, NICE updated its ectopic pregnancy and miscarriage guideline in June 2026 and continues to recommend vaginal micronized progesterone for people who have:
- an intrauterine pregnancy confirmed by ultrasound
- vaginal bleeding during early pregnancy
- a previous miscarriage
NICE found evidence of improved live-birth outcomes in this specific group. It did not find the same benefit for people with early bleeding but no previous miscarriage, or for those with previous miscarriage but no bleeding in the current pregnancy.
Clinical practice can differ between countries and healthcare systems, and progesterone should not be started simply because spotting occurs.
Bleeding in early pregnancy first requires appropriate assessment because ectopic pregnancy and other complications must sometimes be excluded.
Progesterone and Preterm Birth Prevention
This is another area where older online information can be misleading.
Progesterone is not universally recommended for every person who previously delivered prematurely.
Current ACOG guidance states that vaginal progesterone may be considered in a singleton pregnancy when there is a history of spontaneous preterm birth and a shortened cervix.
ACOG also states that vaginal progesterone has not been shown effective for recurrent preterm-birth prevention when the cervix is not shortened.
Intramuscular 17-alpha hydroxyprogesterone caproate (17-OHPC) is no longer recommended for primary prevention of recurrent preterm birth.
ACOG’s current patient guidance also describes vaginal progesterone as an option when a short cervix is identified before 24 weeks in appropriate patients.
The important point is that preterm-birth prevention now depends on pregnancy history, cervical length, gestational age, and other clinical factors—not simply on having had a premature birth before.
Progesterone and Menopause
Progesterone has a different role during menopause than it does during fertility treatment or pregnancy.
As ovarian function declines, both estrogen and progesterone production change.
People may experience hot flashes, night sweats, vaginal changes, sleep problems, and other menopausal symptoms.
When systemic estrogen is prescribed to relieve menopausal symptoms, whether a progestogen is needed depends heavily on whether the person still has a uterus.
Why Progesterone May Be Added to Estrogen
Systemic estrogen stimulates the endometrium.
If someone with an intact uterus takes systemic estrogen without adequate progestogen protection, the endometrium can become excessively stimulated, increasing the risk of endometrial hyperplasia and eventually endometrial cancer.
Adding an appropriate progestogen counteracts much of that estrogen effect.
ACOG therefore recommends combined estrogen-progestogen treatment for many people using systemic menopausal estrogen who still have a uterus.
People who have had a hysterectomy generally do not need progesterone solely for endometrial protection because they no longer have an endometrium.

Progestin

Types of Progesterone Treatment
Oral Micronized Progesterone
Micronization makes progesterone easier for the body to absorb when taken orally.
FDA-approved micronized progesterone capsules are chemically identical to natural ovarian progesterone.
Oral progesterone is used in specific gynecologic and menopausal situations.
One noticeable effect is that it can cause drowsiness or dizziness in some patients. FDA labeling warns that transient dizziness and drowsiness can occur and advises caution with driving or operating machinery.
Vaginal Progesterone
Vaginal formulations are commonly used in reproductive medicine and selected pregnancy situations.
This route delivers progesterone through vaginal tissues and can provide substantial local exposure to reproductive organs.
FDA-approved vaginal products are available for certain assisted-reproduction indications.
Vaginal progesterone is also the formulation addressed by guidelines for selected miscarriage and preterm-birth situations.
Injectable Progesterone
Injectable progesterone is used in some fertility-treatment protocols.
Intramuscular progesterone in oil, for example, may be prescribed as luteal support around embryo transfer.

Some risks frequently described as “progesterone risks” online actually come from studies of combined estrogen-plus-progestogen menopausal therapy, and should not automatically be assigned to progesterone used alone or in a completely different setting.

Does Progesterone Cause Weight Gain?
Progesterone can cause bloating or fluid retention in some people, which may temporarily change how clothes fit or affect the number on a scale.
FDA labeling notes that progesterone can produce some degree of fluid retention.
That is different from proving that progesterone routinely causes substantial long-term body-fat gain.

Someone who experiences marked sleepiness, confusion, severe dizziness, or difficulty functioning after taking prescription progesterone should contact the prescribing clinician.
Do not change the dose or timing on your own unless advised.
Who Needs Extra Caution With Progesterone Therapy?
Whether progesterone is appropriate depends on the indication and medical history.
A clinician may need to consider factors such as:
- unexplained vaginal bleeding
- previous hormone-sensitive cancer
- liver disease
- cardiovascular or clotting history, especially when estrogen is also being used
- pregnancy status
- allergies
- other medicines

One practical example is the capsule formulation.

What Does “Bioidentical Progesterone” Mean?
The word bioidentical is frequently used in hormone marketing.
It generally means that the hormone has the same chemical structure as a hormone produced naturally in the human body.
Micronized prescription progesterone can already meet that definition.
FDA-approved oral micronized progesterone is chemically identical to ovarian progesterone.
So there is no automatic distinction in which:
compounded = bioidentical
and
